Proprietors can use the Membership Form to collect members information and are encouraged to use the NEW Online Membership Google Form to submit registrations. If not submitting online, please forward all hard copy membership forms by mail to the A5PBA Office. It is imperative that ALL the of the information is complete on the hard copy form before submission. An invoice will be sent to you by email once the memberships have been processed.
Membership 2025 – 2026 Fees
Please note the cost of membership for your card:

| Golden Age Ages 55+ as of December 31st | $11.00 |
Bowlers to get your Membership, choose one of the following:
- Use the NEW Online Membership form below, followed by EFT payment.
- Complete the Membership Form and mail to A5PBA Office with cheque payment.
- See your local 5 Pin Association or Lanes Proprietor/Manager to register at your bowling centre.
When using online registration, please note the following stipulations,
- When registering online, EFT payments can be sent to a5pbamemberships@gmail.com at the time of registration. Your registration will not be considered complete until payment is received. Please provide your membership number and bowling centre in the message portion with sending your EFT payment.
- Once the online registration form is submitted, you will receive an email reply with a copy of your submission. Please use this, along with your EFT confirmation email as verification of registration for your zone, if required.
- Please do not send cash in the mail as A5PBA can accept no responsibility for its delivery.
- NO REFUNDS for e-transfer payments.
- Please wait at least five minutes for the email reply as the confirmation may take time if you have a poor internet connection.
