Other Forms

Each Zone is responsible for acquiring, distributing and returning completed forms for each event. Select the proper event and then the relevant form(s). Form Definitions:

• Format is the rules and regulations for the event
• Bowl in Another Zone (Declaration of Intent Form) allows a bowler to bowl in a zone that is not their current residence. This would primarily apply to students and employees who work and bowl in a zone that is not their primary residence . Approval of both zones and the A5 board is required 28 days in advance of the event.
• Each bowler must fill out a Liability Waiver and a Bowler's Medical and History Form
• Zone reports are filled out after zone finals occur.
• The other forms are self explanatory
